Dear Valued Patient,
It is with a heavy heart that I write to you to inform you that due to a family emergency, I will be closing my medical practice in Obstetrics and Gynecology effective 6/20/2025. After 36 years in private practice in Plano this has been a very difficult decision. It has been an honor and a privilege to be involved in the care of each and every one of my patients. I have always considered my patients as part of my extended family.
After 6/20/25 the office will remain open for an additional month for administrative purposes and to fulfill medical record requests. I highly recommend Dr. Eric Jacoby and Dr. Berry Fleming for your continued needs in Obstetrics and Gynecology after the practice closes. They will be available for continuation of care. I have worked with them for over 18 years and I am very confident in their medical abilities, professionalism, and compassion for patients. They are conveniently located across the hall from me in the same building, 3108 Midway Rd, suite 201. Their phone number is 972-473-2020, website- pwhcare.com. Of course it is your option to choose care from another physician. Copies of records can be transferred to them or another physician or to the patient by request. There will be a charge for copying medical records based on the Texas Medical Association recommendations.

It will be much easier to obtain the records over the next 7 weeks while the office is still open. After this time we will store the records and you can contact us at the above email. Requests will be filled as per time limitations dictated by state law retention requirements. There will be a charge for medical records based on the Texas Medical Association recommendations.
Once again it’s been the honor of my life to be involved in your care and I wish you the greatest happiness and health in the future.
